Meet The Autumn Park Team

Our 24-hour caring and specially trained staff not only administers a full range of services for our residents, but considers them part of the Autumn Park family. You will often see our Executive Director sharing stories with residents in the dining room, our Life Enrichment Coordinator playing cards, Wii bowling or watching a movie with residents in the activity center or our Chef coordinating special snacks or events for residents in the private dining room. This is how the Autumn Park family knows when our residents had a grandchild, expect a visitor for lunch or just need a light bulb replaced. Our team encourages resident feedback to develop programs that will complement our resident's interests and help them cultivate new ones.

Eileen Thomas, HFA

Executive Director
Eileen has been the Executive Director at Autumn Park since 2009. She is responsible for overseeing daily operations of the community, ensuring high quality care and services are provided to residents and achieving high resident satisfaction. Eileen has been in the health care industry since 1992. She started her career in long term care as a Certified Nursing Assistant / Qualified Medication Aide. Eileen also spent 9 years working for Chrysler Corporation. She attended HOPE (Hoosier Owners & Providers for the Elderly). She serves alongside her husband as the Youth Group Leader for their Church. Eileen is the mother of 4 children and 1 lovely daughter-in-law. Eileen’s favorite pastime is spending time with her family, her extended family at Autumn Park and taking long trips on her Harley Davidson. Eileen believes strongly that senior care is her calling in life, along with being a wife and mother.

Lori Cameron

Dietary Service Director
Lori has been the Dietary Service Director at Autumn Park since 2010. She is responsible for overseeing the Dietary Department and makes delicious and nutritious home-cooked meals from scratch every day for our residents. Our residents enjoy a five-star dining experience lead by Lori filled with a rich assortment of menu options. In collaboration with our Registered Dietician, Lori incorporates resident’s personal recipes, special requests, old time favorites and specific dietary needs so our well-balanced menus reflect their health, tastes and preferences. Understanding how important great food is to our residents, Lori is consistently introducing exciting new cuisines for the community to enjoy. Lori has been in the food industry for 20 years. She has been the Chef at other senior living communities and ran a gourmet catering company. Lori attended Parkland College. She is ServSafe certified.

Dawn Broomfield, LPN

Wellness Director
Dawn has been the Wellness Director at Autumn Park since 2011. She is responsible for overseeing resident health care services, the medication delivery system and in-service training of the Health Services Department. Dawn has worked in the health care industry for over 21 years. She worked for our sister community, Autumn Glen, overseeing resident care before being promoted to her current position. Dawn previously held numerous positions at assisted living and skilled nursing / rehabilitation facilities throughout Indiana including Assistant Director of Nursing, Charge Nurse, In-Service Director and Unit Manager. She led her groups to multiple perfect State surveys. Throughout her distinguished career, Dawn received a variety of awards including “Employee of the Month” and “Excellence Award in Recognition of Outstanding Service to the Hospice Community” presented by the Vista Foundation.  She has also received a “Certificate for Leadership Skills” and a “Certificate of Appreciation” from John P. Howe, III, M.D. of Project Hope recognizing her commitment to providing health care services to the less fortunate.  Dawn earned her nursing degree in 1989 from Jefferson Lewis College in New York. While attending nursing school, she received the Certificate of Recognition for Clinical Performance in Nursing. Dawn resides in Indianapolis with her husband and 5 children, and enjoys photography and scrapbooking.

Rhonda Berns

Life Enrichment Coordinator
Rhonda has been the Life Enrichment Coordinator at Autumn Park since 2007. She is responsible for planning and coordinating community-sponsored activity, exercise and wellness programs for residents. Rhonda has been in the health care industry for 10 years and has extensive experience motivating, working with and encouraging socialization for residents requiring all levels of care. She completed specialized training and was certified as an Activity Director in 2004. Before joining the Autumn Park family, Rhonda was the activity director and programmer for an Indianapolis skilled nursing facility and lead courses on Alzheimer’s and related memory disorders. Rhonda previously owned her own businesses including a furniture store and gourmet coffee shop. She also managed a gift and candy store and loved sending sweets and gifts on special occasions for her customers. Rhonda attended Indiana University and earned her degree in Elementary Education with an endorsement in Special Education in 1973.  She volunteers at other assisted living and nursing homes and was the co-chair of fundraising for the Indianapolis Hebrew Congregation. She was presented with Indianapolis Star’s special “Why I like My Job” award. Rhonda and her husband reside in Indianapolis and have 3 lovely daughters and 4 grandchildren. She enjoys golf and needlepoint.

Amanda Mitchell

Community Relations Coordinator
Amanda has been the Community Relations Coordinator at Autumn Park since 2011. She is responsible for sales and marketing and has helped numerous families find placement for their loved ones. Amanda has extensive experience in health care. Previously, she ran marketing for an Indiana-based home health care company and worked for CICOA Aging and in Home Solutions ensuring her senior clients remained safely in their homes. Amanda also oversaw sales and marketing for an assisted living and memory care community in Indianapolis and was promoted to direct the Alzheimer’s program. She received her bachelor’s degree in Gerontology from Bowling Green. She is a member of CICMSA and is on the committee for the annual CMSA conference.  Amanda resides in Indianapolis with her husband.  She enjoys the Pittsburg Steelers football and spending time with her family and friends.

James Hale

Maintenance Director
“Jim” has been the Maintenance Director at Autumn Park since 2010. He is responsible for maintenance, repairs and improvements to our residents’ home. Jim loves working and interacting with our residents and families on a daily basis to ensure our residents are safe and happy. Jim’s extensive 25 year career working with logistic providers as a senior configuration assembler, running tool distribution warehouses, leading pharmaceutical wholesale departments and as a residential / commercial installation mechanic for heating and air conditioning companies makes him a tremendous asset to Autumn Park residents.  Jim received certifications in HVAC from Ferris State University. Jim attended Ivy Tech College and Valparaiso where he studied HVAC and welding. Jim resides in Indianapolis with his wife and enjoys hiking, photography and racing.

Teresa Stephens

Housekeeping Director
“Terri” has been the Housekeeping Director at Autumn Park since 2009. She is responsible for keeping our resident’s homes and surroundings in a clean and safe condition. Terri is a self-motivated achiever who simply gets the job done and loves the residents of Autumn Park. Prior to Autumn Park, Terri was a certified Nursing Assistant for 12 years.  She has 2 children and enjoys spending her free time with her family. Terri is 1 of 7 children and her older sister is the Housekeeping Director at our sister community in Indianapolis, Autumn Glen.

Dinetta Tiffany White

Office Manager
“Tiffany” has been the Office Manager at Autumn Park since 2009. She is responsible for maintaining day-to-day business office operations and procedures, including payroll, rent deposits and resident billing. Tiffany is easily recognizable by her big smile and cheerful attitude. For the previous two decades, Tiffany owned and managed a wide spectrum of businesses including salons, hotels and an automobile finance company. She attended First Business School in Chicago, Illinois and received a certificate in business management. Tiffany resides in Indianapolis and has a son and two gorgeous grandchildren. She was extremely close with her grandmother and shares a strong love for children and the elderly. She is very active in the Girl Scouts and enjoys roller-skating.
